| The Federal Verification Worksheet should be completed by families of prospective and returning students who wish to apply for need-based assistance. The Verification Worksheet requests information about the household, tax filing status and untaxed income. This information will be used in determining eligibility for both federal and institutional assistance, as well as completing federal verification requirements should the student be selected. |

| This form should be submitted either as part of the IDOC packet or returned to the Office
of Financial Aid by mail. This form will not be accepted
by fax or e-mail. Please be sure to complete the form for the appropriate academic year. |
