Business/Farm Supplement
Description: The Business/Farm Supplement is used to gather additional information from families who own a business or a farm.
Audience: Families of students applying to Northwestern as entering freshmen or transfer students who indicate ownership of a business or farm on their CSS Profile must complete this form. Current students already attending Northwestern do not need to complete this form if they have done so in a prior year.
Instructions: The Business/Farm Supplement may ask for information that does not apply to your particular enterprise. If one of the questions is not applicable, put a zero where appropriate. Do not leave questions blank. Feel free to use the space provided on the form or attach a separate sheet if you wish to elaborate on any of your answers or to explain why some parts of the form are not applicable. Complete the form for the appropriate calendar year and mail it to the Office of Financial Aid; it will not be accepted by fax or e-mail.

Health Insurance Assistance Request Form
Description: The Request Form is used to apply for the Health Insurance Assistance Program, which alleviates the cost of the University's required health insurance program for undergraduate students with high financial aid and no other form of health insurance coverage.
Audience: Students with a total Expected Family Contribution of $5,500 or less, as indicated on the most recent financial aid award announcement.
Instructions: If you feel that you qualify, complete the Health Insurance Assistance Request Form and submit it to the Office of Financial Aid either prior to or during the fall quarter. Requests submitted during winter or spring terms may also be considered, but the insurance charge is assessed during fall term.
If the assistance is granted, your financial aid award will be increased by the cost of the University Student Hospitalization Plan ($2,360 for 2008-09), and this credit will offset the Plan charge on your invoice. This increased assistance may be in the form of a scholarship or loan, depending upon availability of funds. Any assistance granted is contingent upon Plan enrollment.

Loss of Wages Statement
Description: The Loss of Wages/Employment Statement is used to show a family's actual and anticipated income for the calendar year when one or both parents have experienced a reduction in wages and/or are currently unemployed.
Audience: A family only needs to file this form if specifically requested by our office. It will be requested if a family is appealing their financial aid decision due to a recent loss of employment or reduction of income, or if additional information is needed to thoroughly review an initial application.
Instructions: Fill out the form, answering "N/A" for questions that are not applicable. We encourage you to write a letter elaborating on your circumstances and attach it to this form along with the following required documentation:
- Termination letter or letter verifying reduction in salary
- Documentation of severance package and accrued vacation/sick days (if applicable)
- Documentation of unemployment benefits received
- Last paycheck stub showing year-to-date earnings (for both parents)
- Monthly Expense Statement
Mail the completed form and supporting documentation to the Office of Financial Aid; it will not be accepted by fax or e-mail.

Monthly Expense Statement
Description: The Monthly Expense Statement is used to show a family's cash flow and some of the extra expenses that a family may incur.
Audience: A family only needs to file this form if specifically requested by our office. It will be requested if a family is appealing their financial aid decision due to new circumstances, or if additional information is needed to thoroughly review an initial application.
Instructions: Fill out the form, answering "N/A" for questions that are not applicable. Feel free to use the space provided or attach a separate sheet if you wish to elaborate on any of your answers. Mail the completed form to the Office of Financial Aid; it will not be accepted by fax or e-mail.

Natural Disaster Relief Form
Description: The Natural Disaster Relief Form is used to report a change in financial circumstances and request additional assistance when a family has been affected by a natural disaster.
Audience: Families who have experienced a change in financial circumstances due to a natural disaster.
Instructions: Fill out the form. We encourage you to write a letter elaborating on your circumstances and attach it to this form along with the following required documentation:
- Federal tax returns with W-2 wage statements (if not on file)
- Documentation of the date that the natural disaster resulted in loss of employment
- Documentation of Official Declaration of Natural Disaster status
Mail the completed form and supporting documentation to the Office of Financial Aid. This form will not be accepted by fax or e-mail.

Noncustodial Parent's Statement
Description: The Noncustodial Parent's Statement is used to determine eligibility for University assistance if a family is applying for need-based financial aid and the student's parents are divorced, separated, or have never been married. Both parents, regardless of their current marital status, have the primary responsibility for providing for their child's education.
Audience: Families of students applying to Northwestern as entering freshman or transfer students who indicate a noncustodial parent on their CSS Profile must have the noncustodial parent complete this confidential form. Current Northwestern students already attending Northwestern do not need to have this form completed if they have done so in previous years.
Instructions: Fill out the form, answering "N/A" for questions that are not applicable. Feel free to use the space provided or attach a separate sheet if you wish to explain any special circumstances. Complete the form for the appropriate academic year, and mail it to the Office of Financial Aid; it will not be accepted by fax or e-mail.
