| The Noncustodial Parent's Statement
is used to determine eligibility for University assistance.
Northwestern believes that both parents, regardless of their
current marital status, have the primary responsibility for
providing for their child's education. Completion of this form
will be required if a family is applying for need-based financial
assistance and the student's parents are divorced, separated,
or have never been married. |

| Families of students applying
to Northwestern as entering freshman or transfer students
who indicate a noncustodial parent on their CSS Profile must
have the noncustodial parent complete this confidential form.
Current Northwestern students already attending Northwestern
do not need to have this form completed if they have done so
in previous years. |

| If one of the questions is not
applicable, please put an "N/A" where appropriate. Feel free
to use the space provided or attach a separate sheet if you
wish to explain any special circumstances. Be sure to complete
the form for the appropriate academic year, and to return it
to the Office
of Financial Aid by mail. This form will not be accepted
by fax or e-mail. |
